Text field Matrix with Textfield as Answer Topic/Dimension

Hello,

I want to use a text field matrix, where also the answers/dimensions are open text fields. So that the respondents themselves can define/fill in the answer to which they use the scale. Please have a look at the screenshot. I would like to programme the fields marked there as text fields so that the respondents can also fill in their answers there themselves.

I assume that I need to programm the question in html myself… Does anyone already have this code? Or can anyone provide me with the html code of a text field matrix so that I can edit it myself?
